Abstract: Mental health brings at its heart, innumerable stigmas and prejudices that can generate diverse barriers of access to the patients that need the resolutive attendance in the Basic Attention. The Family Health Strategy (ESF) proposes comprehensive care to people in its community, one of its assumptions being networking and intersectoriality. Regarding mental health care, the elucidation of the barriers to care is essential for the improvement of the services and access of patients to the treatment that encompasses their context of suffering, not just their diagnosis. Thus, the present research had as objective to know the perception and the practical reality in the action of the professionals of Basic Attention in the attention to Mental Health in the municipality of Itumbiara (GO). This is a cross-sectional study of the type of quantitative and qualitative survey carried out on professionals from the 20 ESF teams from the municipality of Itumbiara (GO) from September 2018 to January 2019. The questionnaire consisted of 20 questions on a scale of type Likert and 5 questions opened from a semi-structured roadmap. Likert scale questions were evaluated according to the frequency and distribution of proportions, and the score was transformed to a scale from zero to 100, zero related to the worst evaluation, and 100 to the best evaluation. The open questions were evaluated through the Bardin content analysis using the IRAMUTEQ software. Among the 174 professionals interviewed, 23.0% of the professionals agreed or fully agreed that they felt able to work with mental health in the FHT (n = 40), 37.9% that their team received mental health support (n = 82 ) and 37.5% that their team was able to organize care according to the unique demand of each patient (n = 64). Regarding difficulty in providing care to patients in Health, only 29.1% disagreed or totally disagreed (n = 48). The mean of the transformed score of all questions was 52.0 ± 13.7. The analyzed content of the open questions could be categorized into four classes according to the percentages of themes, being the words that present the most outstanding: people, health, mental, difficulty, patient and knowledge. It can be noticed from the speeches that the contact with patients and family members has been an important aggravating factor in the relationship between the FHT and the subject to which the care is intended, expressing the difficulty of the professionals not to feel able to deal with this demand. It can also be inferred that family health workers perceive themselves lacking matrix support to be carried out by the mental health reference unit of the municipality, realizing that their performance in the FHS is limited. Even though they present difficulties in their work, professionals are able to perceive their importance in the reality of the subjects with whom they relate and recognize the need for more preparation, more knowledge and new skills. In this way, it was possible to perceive the different degrees of difficulty both in the perception and in the practice of the workers, which allowed comparison with the sociological analysis of Giddens, evidencing that the routine is not created by the individual, but reproduced by him. Its criticality may determine that the structure remains or there may be a possibility of change.

Abstract: Attention Deficit Hyperactivity Disorder (ADHD) is one of the leading and most neglected mental disorders. It is associated with unsatisfactory school performance, conflicting social relationships, and low self-esteem. These symptoms tend to contribute significantly to the emergence of problems such as school expulsion, drug abuse, trafficrelated mortality and involvement with crime. There is little national literature on the approach to ADHD in Primary Care. Some international experiences shed light on the subject, but with no homogeneity. The present study analyzes the potentials and limitations of primary care in diagnosing and treating schoolchildren with ADHD. For registration, systematization and analysis of the material that constitutes the object of this study, qualitative methodology was adopted. A total of 05 cases of schoolchildren with a diagnosis of ADHD were selected. They were all treated at the Primary Care Clinic Vila Basevi. The medical records were analyzed and supplemented with semi-structured interviews with parents and teachers, as well as the matrix support psychiatrist. Systematization was carried out using narrative medicine. Thematic analysis was applied, using the essential attributes of primary care as categories: access, longitudinality, completeness and coordination of care. From this study, it was possible to design a flow of care and outline specific techniques for treating school children with ADHD in primary care, involving aspects inherent to team and community-based work. There are natural limitations to such a pioneer study. It is therefore clear that new research initiatives are needed to increase knowledge on the subject as well as to improve primary care resolutivity.

Abstract: Today's rural territory has dynamic and complex specificities, and its landscapes and populations change rapidly, in communication with the urban environment. Still, people living in rural areas experience everyday difficulties with geographical isolation and the scarce provision of public and private services in these places. In this context, women are vulnerable, not merely because of the biological processes characteristic of life-cycle stages, but because of the social and economic implications that restrict the possibilities for affirmation and autonomous construction of female identity. Having integrality in perspective, this research aimed to recognize needs and demands in health of women users of the Family Health Strategy, in the community of Padre Lúcio, state of Goiás, in Brazil. Two focus groups and five individual depth interviews were conducted, with women of different ages and circumstances. Focus groups and interviews were recorded on electronic media. Later, their products were transcribed and subjected to content analysis. The following demands were identified: prescription of benzodiazepines; surgical sterilization; and hormone replacement therapy. As needs, were highlighted: health education; family planning; mental health approaches; sociocultural activities; educational activities for development skills; educational activities for information on social rights; and implementation of pharmacy, collection of laboratory tests and community gym in the physical structure of the family health service. This research had as reference the theoretical-philosophical constructs that guide health care, related to the autonomy and dignity of the subjects in the elaboration of their life projects. Dialogic processes and mobilization of social networks were valued as tools of this care that is understood as emancipating. Further research is suggested through men's involvement in care processes, overcoming the cultural restriction of primary care as a women's space, and seeking to understand the implication of masculinities in illness and violence, from a gender relational perspective.

    <title>Hemovigilância e segurança do paciente: análise de reações transfusionais imediatas em idosos</title>

    <summary type="text">Título: Hemovigilância e segurança do paciente: análise de reações transfusionais imediatas em idosos

Abstract: Objective: To identify immediate transfusion reactions in elders hospitalized in a public hospital in the Federal District. Methods: This is an observational, retrospective, documental, and analytical research, with a quantitative analysis of 516 transfusions of packed red blood cells in elders hospitalized in the largest public hospital, who required blood components, from June to December 2017, through descriptive statistics. Results: The sample  corresponded to 46.36% of the total number of  transfusions in elders in the period. The mean age was 70 years old. There were adverse effects (reactions to the transfusion) in 12 (2.3%) transfusions. Respiratory alterations (33.3%) and fever (23.8%) were the most common events. Conclusion: The incidence of reactions to the transfusion is below national and international rates, indicating probable undernotification, which could be associated to a lack of knowledge regarding its clinical manifestations and the lack of systematic monitoring of the transfusion.
